Registration

To register your student at one of the schools in our district, please stop by the school’s office and pick up a registration packet. The packet contains forms requiried information such as:

  • Home Address
  • Home or Work Phone
  • Parents’ Names
  • Emergency Contact Information
  • Home Language Survey Form
  • Student Medical Information

In addition, please supply the following information and documents:

  • Student Birth Certificate
  • Immunization Record
  • Picture ID of parent registering student
  • Proof of Residency (e.g. a utility bill, rental agreement, etc.)
  • Transcripts
  • Withdrawal Slip

Please fill out all forms and return them to the school’s office, and remember to bring the above documentation with you.
